Anticonvulsants are the medications used in the treatment of epileptic conditions. There are various categories of anticonvulsant which includes hydantoins, barbiturates, benzodiazepines. Their use is also extended to treat bipolar disorders, borderline personality disorders as many are used in mood stabilization and reducing neuropathic pain. These drugs are known to cause various adverse effects depending on dose. Antiepileptic drugs not only cause adverse effects related to central nervous system but also cause cutaneous symptoms. Some of the cutaneous adverse symptoms related to antiepileptic medications are toxic epidermal necrolysis, Stevens-Johnson syndrome, acute generalized exanthematous pustulosis, drug induced hypersensitivity syndrome (DIHS), drug reaction with eosinophilia and systemic symptoms (DRESS). Epidermolysis bullosa aquista (Eba) is a rare chronic auto immune disease that causes skin blisters. This blister may appear to minor injury, rubbing or even from heat exposure. Eba is a subepidermal disease with an incidence of 0.1 new cases per million per year. Eba is characterized by autoimmunity to type VII collagen located at dermis and epidermis junction that alters the normal functioning of anchoring fibrils. Eba has no permanent cure and treatment mainly focus on caring for blisters and prevent new ones. Some treatment success has been with corticosteroids, dapsone, plasmaphoresis, photopheresis....
Hydrocephalus is defined as an increase volume of intracranial cerebrospinal fluid (CSF) due to excessive accumulation resulting from impaired circulation and absorption due to excessive production of CSF from choroid plexus. A female infant at 6 months of age, child was brought by her parents with complaints of increased head size since 1 month. Physical examination revealed severe hydrocephalus with head circumference of 46 cm and length 62 cm. Ultrasound examination confirmed bilateral ventriculomegaly. VP shunt (ventriculo peritoneal shunting) was placed to shunt the CSF fluid to peritoneum and haemostasis is secured. The earlier this disorder is diagnosed, the better the chance for successful treatment. Hence, early diagnosis and regular monitoring after surgery is important in patients with hydrocephalus. All efforts should be made for long term follow-up to treat this frequent otherwise dramatic condition....
Hailey Hailey disease (HHD) is a Beningn familial rare autosomal dominant disorder. The main cause of HHD is due to mutations in ATPC21 gene which encodes the human secretary pathway ca2+/mn2+ ATPase protein. Faulty calcium pump action leads to disorganized function of desmogleins which are calcium dependent adherence proteins. A 61 year old male patient admitted in our hospital in dermatology ward with chief complaints of itchy painful lesions skin lesions over groin and right axilla since 15 days. Report shows well defined vesicular, pustular, plagues over inner aspect of thighs, right axilla. Patient was treated with T. Prednisolone 20 mg, T. Cetirizine, T. Hydrochlorthiazide, T. BC, Vit C. Patient was discharged and advised to continue T. Prednisolone 20 mg OD, mupirocin cream. No reported cure is present but corticosteroids, topical antibiotics, antifungals useful in management of HHD in many cases....
